In the event a parent/ guardian cannot be reached, who should we contact?
Required for any event on or off church property.
Does your child have any type of medical, physical, or mental conditions that leaders should be aware of? If so, please explain.
I, parent/guardian of the above child/ren, understand all reasonable safety precautions will be taken at all times by First Presbyterian Church of Ambler and its agents during Youth Ministry Programs. I/We understand the possibility of unforeseen hazards and know the inherent possibility of risk. I/We agree not to hold First Presbyterian Church of Ambler, its leaders, employees and volunteer staff liable for damages, losses or injuries incurred by the subject/s of this form.
It is okay if we use pictures of your child in Church-sponsored publications outside of the Church building?
In case of an emergency, I hereby give my permission to First Presbyterian Church of Ambler sponsors of events to permit emergency or hospital personnel and/or a licensed physician or health practitioner to perform emergency treatments and administer drugs in conjunction with such emergency treatment. I understand that FPC Ambler sponsors of each event will determine whether emergency care is necessary, arranging for such services to be provided, and understand they will contact me as soon as possible.
My child has permission to attend all church sponsored youth activities as listed in calendars, on social media, through texts, and any other publications.
I acknowledge these rules or conduct expected from each participant and parent: Respect one another, staff, and adult leaders, respect property, no lighter permitted, no fighting, weapons, fireworks, explosives, no students permitted to drive for events, participation with the group expected, no offensive or immodest clothing, no alcohol, no drugs, no tobacco.
I and my child acknowledge that misconduct may result in transportation home from any activity at the parent’s expense. My child and I agree to follow the instruction of the pastor, leader, or volunteer who has been delegated leadership authority.
