VBS Hours:
6-8 pm. (M-F)
All activities held on church grounds.

Family Dinner Hours:
5-6 pm. (M-F)
(All children must be accompanied by an authorized adult during VBS Family Dinners.)

Parent Release

As parent/legal guardian of the above named child/children listed above, I give my permission for the child/children to be involved in the overall activiities of VBS Summer Friends. I/We understand First Presbyterian Church of Ambler (FPCA) will attempt to take reasonable precautions during VBS Summer Friends, but understand the possibility of unforeseen hazards and know the inherent possibility of risk in all activivities. I/We agree not to hold FPCA its leaders, employess, and volunteers liable for damages, losses or injuries incurred by the subject of this form.

Medical Release Permission

In the case of an emergency, I hereby give my permission to First Presbyterian Church of Ambler (FPCA) staff and volunteers to permit emergency or hospital personnel and/or a licensed physician or health practitioner to perform emergency treatments and administer drugs in conjunction with such emergency treatment. I understand that FPCA staff and volunteers of each event will determine whether emergency care is necessary, arranging for such services to be provided, and understand they will contact me as soon as is practical.

Daily Check In/Check Out:

All dinner and VBS participants: Please enter the church through the doors by the municipal parking lot (Cavalier Drive).

VBS Sign-In will start at 5:50 pm in the church cafe.

Parents/guardians are invited to relax in our church's Parent Zone while their children attend VBS (6-8 pm).

Parents/guardians are not required to remain on church grounds during VBS (6-8 pm).

Children not attending VBS will remain under parent/guardian's supervision while on church grounds.

VBS Sign-Out/Pick Up: Pick up/sign out children in the church cafe. We will dismiss at 8 pm.

Approved Pick Up:

Children will be released to parents/guardians listed on this form or other pre-approved adults. Below, please list adults other than yourself who have your approval to pick up your child/children on this form.
